Healthy Rewards is a voluntary program designed to reward colleagues and spouses for taking an active role in their health. All colleagues and spouses covered on the Saint Francis Healthcare System medical insurance plan are eligible to participate. See below for the opportunities to choose your level of participation.

Level 1
Complete after Jan 6, 2025

Sign in to ManageWell
Sign in at managewell.com and agree to the 2025 ManageWell website/app terms after January 6, 2025 to receive the Level 1 $5 monthly premium reimbursement.

Level 2
Due by Feb 28, 2025 (to be eligible for Level 3)

Complete an annual preventive exam and labs with your provider. Have your provider submit a Healthy Rewards Ambulatory Referral to receive the Level 2 $5 monthly premium reimbursement.

Level 3
Level 1 & 2 must be completed to be eligible for Level 3

Participate in monthly activities to be eligible to earn the Level 3 10-20% monthly premium reimbursement.

How do I log in to my ManageWell account?

Returning Healthy Rewards Participant: Sign in at managewell.com.

First-Time Healthy Rewards Participant:

  1. Visit managewell.com from your computer web browser. You can also download the free App (ManageWell 2.0) and follow the prompts to complete your online registration from your smart phone.
  2. Enter the email you wish to use for your ManageWell login.
  3. If you are an employee, enter the initials of Saint Francis Medical Center (SFMC) followed by your employee number. Example: SFMC12345 If you are a spouse, enter the primary covered colleague’s employee number followed by the letters SO. Example: SFMC12345SO
  4. Enter YOUR date of birth, as this will be tied to YOUR personal account.
  5. Read through the Healthy Rewards Participant’s Rights, Responsibilities and Program Disclosures. Once you acknowledge the disclosure it will take you to the home page of ManageWell.

Once you are logged in to your ManageWell account, select “Rewards/Messages/Goals”. You will see a golden chalice if you have completed the Level 1 and/or Level 2 requirements. You must complete Level 1 & 2 enrollment to be eligible to participate in Level 3 each program year.

Who is assigned a Health Coach?

Health coaching is available to all Level 3 participants free of cost. Those that are on a Chronic track in Healthy Rewards are assigned a Health Coach. This telephonic based program will help you understand how lifestyle changes can help you reach your health goals. Trained coaches will provide you with the tools and support you need to make changes in your lifestyle that will help you reduce risks and improve your health. When you work with a coach, together you will create an action plan and set realistic goals to help you make behavior changes. To find the Health Coach assigned to you, visit your ManageWell portal.

How do I earn my monthly 10-20% insurance premium reimbursement?

Your monthly reimbursement will also be referred to as your “monthly reward”. To earn your Level 3 reward each month, you must complete the activities in your ManageWell account each month. Click on “Learn More” next to the current month to see the activities for the current month. As activities are completed a green checkmark will appear. Once you have completed all required activities, the chalice for the month with turn golden.

How is my insurance premium reimbursement (10% vs 20%) determined?

This is determined by the eligible members on your SFHS insurance plan.

  • If it just you on your insurance plan, then you will earn a 20% premium reimbursement each month that you complete all Level 3 requirements.
  • If a spouse is on your insurance plan, he/she is also eligible to participate in Healthy Rewards. You EACH can earn a 10% premium reimbursement each month that you each complete all Level 3 requirements.

The dollar amount that the 10-20% reimbursement equates to on your paycheck depends on your personal medical insurance plan.

What if I feel that the requirements are not appropriate for me?

Healthy Rewards is committed to helping you achieve your best health. If you believe that an existing or newly developed health issue may make it unreasonably difficult or medically inadvisable for you to attempt achievement of a goal established in Healthy Rewards, you are encouraged to request a Reasonable Alternative Form. Your primary care provider must complete the Medical Condition Statement summarizing your health condition so that a reasonable alternate behavioral health goal may be developed for you.
